Lupus Nephritis (LN) is one of the most severe organ manifestations of systemic lupus erythematosus (SLE), an autoimmune disorder marked by loss of immune tolerance to endogenous nuclear material, resulting in systemic autoimmunity that damages various tissues and organs. The overall incidence and prevalence of SLE range from 7.4 to 159.4 cases per 100,000 people and from 1.4% to 21.9%, respectively. According to the National Kidney Foundation, lupus nephritis is a form of glomerulonephritis that causes inflammation of the small blood vessels that filter waste in the kidney (glomeruli), and sometimes the kidneys themselves; histologically, LN is classified into six distinct classes representing different manifestations and severity of renal involvement in SLE.
Lupus nephritis can cause permanent kidney damage, known as chronic kidney disease (CKD). The most serious type, proliferative nephritis, can cause scarring of the kidneys that prevents them from working properly, and chronic kidney disease that worsens to the point the kidneys stop working is called kidney failure or end-stage renal disease (ESRD). Major risk factors associated with LN include gender, ethnicity, genetic factors, and environmental factors, and it is typically treated with drugs that block the body’s immune system, including glucocorticoids and immunosuppressive agents such as cyclophosphamide or mycophenolate mofetil (MMF).
A variety of pathogenic mechanisms are involved in the development of lupus nephritis, with its extra-renal etiology based on a combination of genetic variations that alter the mechanisms normally ensuring immune tolerance to nuclear autoantigens. As renal immunopathology progresses, tertiary lymphoid tissue formation and local antibody production add to intrarenal complement activation. Diagnosing lupus nephritis requires clinicians to maintain a high index of suspicion in patients with known SLE and worsening renal function; persistent eosinophilia can also occur, and patients with SLE need to be routinely tested for early detection of renal impairment.
Treatment of lupus nephritis aims to reduce symptoms, prevent disease progression, and maintain kidney function; there is no cure, but various treatments help achieve these goals. Medications such as steroids (prednisone), immunosuppressive drugs (cyclophosphamide, mycophenolate mofetil), and monoclonal antibodies (rituximab, belimumab) are commonly prescribed, alongside supportive treatments such as diet changes, blood pressure medications (ACE inhibitors, ARBs), and diuretics to manage symptoms and protect kidney function.
